Practitioner acknowledges that, at their request, Abbott Diabetes Care (ADC), located at 1360 South Loop Road, Alameda, CA 94502, is providing FreeStyle Libre product(s) at no charge to Practitioner solely for distribution to patients in accordance with the indications for use.

Practitioner represents and warrants that they are licensed by applicable state law(s) to use or order the use of prescription medical devices in the course of their professional practice. To the extent that the Practitioner’s prescribing authority is predicated on meeting additional state requirements, including, but not limited to, certification from the relevant professional board authorizing prescriptive authority or a collaboration/supervision agreement with a physician that addresses prescriptive authority, they represent and warrant that they comply with any additional requirements.

Practitioner certifies that they will not resell or trade the no-charge products(s) and will not seek reimbursement from any patient or third-party payor, including federal health care programs. If the services provided by the Practitioner are covered under global payment systems (such as diagnosis-related groups), then the no-charge product(s) must be treated appropriately under the billing procedures applicable to the payment system.

Practitioner agrees to discard/dispose of or return any unused FreeStyle Libre no-charge product(s) in their possession after its expiration date.

Practitioner certifies that they are aware that the FreeStyle Libre no-charge product(s) are not approved for inpatient use or for patients in acute care who are critically ill and is provided by ADC for intended use in accordance with product labeling.

Who is eligible for web sampling?

Prescribing healthcare providers may request Libre sensor samples. Government employees are not eligible to receive sensor samples from Abbott. These sensor samples are intended for patients living with diabetes. If you currently have a FreeStyle Libre sales representative, please reach out directly to them for sensor samples. You can connect with a sales rep here.

How can I get more samples?

After receiving sensor samples, please sign and submit your packing slip. After signing, you can place another order for samples using the online form.

You can also opt in for recurring sensor sample shipments until the end of the year. Sensor samples will be shipped on a recurring schedule directly to your clinic address. To sign up for recurring sensor samples, please click the checkbox confirming auto-shipment when consenting to the Terms and Conditions.

The FreeStyle Libre 3 system includes the Libre 3 Plus sensor, Libre 3 sensor, Libre 3 app, Libre app, and the Libre 3 reader.

FreeStyle Libre 3 system is cleared to be used by children 4 years and older with Libre 3 sensor and 2 years and older with Libre 3 Plus sensor.
